Key Fob Replacement

THE RULES:

Please read the following rules prior to completing this form:

The new fob must be picked up at the front desk before 4:00 pm for a same-day evening class and before 8:00 am for a morning class start time.
You may contact the office to discuss alternate options for pick up at 515-985-1160.

  • Daytime Students

    1) Apprentices MUST purchase and pick up a replacement fob before 8:00 AM morning class start time.

    2) The purchase of this new fob inactivates any previous fob under your name.

    3) Purchasing a fob via this site, does not confirm a clock in or a clock out time for a class.

  • Evening Students

    1) Apprentices MUST purchase and pick up a replacement fob before 4:00PM for a same-day evening class.

    2) The purchase of this new fob inactivates any previous fob under your name.
